Please make sure you have the following information before making a booking

• Your email address. If you do not have an email address, you can provide the email of your emergency contact or renal unit
• Emergency contact details including name, phone number, and email address
• Medicare details
• Your dialysis unit details including the address, phone number, and email address
• Your nephrologist's and GP's name and phone number

To make your booking:

1. Select your preferred State to see available locations
2. Select your preferred Destination to see available times
3. Select your preferred Time (7:00 AM* for Morning Sessions and 2:00 PM* for Afternoon Sessions)
   *Note: These are only indicative times, and you will be confirmed by the respective hospital for actual session timings
4. Click on More Times for more available sessions in that location
5. Choose a session by clicking on "Add this time..." repeat until you have added all the sessions you wish to attend
6. Once you have selected the final session you want to book for this holiday click the red button "Finish selecting times and confirm"
7. Fill in your form details and confirm your selected appointments.
8. Once booked, you will be contacted by respective hospital to confirm your booking timings.
