Transitioning to adult care
If you are a young person being treated at a children's hospital, you will eventually need to be transferred to an adult health care setting. This usually happens around 18 years of age.

State of the Nation Summary Report
In 2018 Kidney Health Australia received federal government funding to develop and pilot a youth program to engage and support young people (15-24 years) living with kidney disease. A National Consultation Process was conducted over 12 months to identify the major needs of this cohort and involved young people with kidney disease, their families and carers and leading renal healthcare professionals.
Transition and Transfer
Moving from paediatric to adult care is tough. This booklet is a useful resource for children of 12-14 years and above and their families and includes information, tips and references about the process of transition and transfer.
Consensus Statement
Following the National Consultation Process in 2018, a Consensus Statement was developed to supplement and support the State of the Nation Report with recommendations for best practice clinical care for the successful transition and transfer of young adults to adult health care.
